Plan review

End of plan review

  • Nearing the end of a participant’s NDIS plan, the participant can book in for a scheduled plan review meeting with their LAC or Planner within 100 days of their plan end date.

  • The SC assists the participant in being prepared for that review by reviewing their goals and making new ones with them if necessary for the next plan.

  • The SC writes an end of plan progress report by using the template the NDIS provides.

  • The SC gathers and reviews all supporting documents and assessments from the providers that the participant has been engaging with (Occupational therapist, psychologist, GP reports, etc.) and uses them in the end of plan report to justify the request for further reasonable and necessary supports within the participant’s next NDIS plan.

  • SC submits this report before the scheduled plan review meeting so the planner/ LAC can review it.

  • SC attends the plan review meeting with the participant and assists with providing any information during the meeting (currently all review meetings are conducted over the phone due to the pandemic and the NDIA will call the participant and Support Coordinator at the same time).

Plan renewal

  • Once the plan review is over, the planner will formulate a new plan and send it to the participant. If the participant wishes, they can request that a copy of the plan is also sent to the Support Coordinator so they can continue support with them.
