GSC explores support options with the participant for each funding category in their NDIS plan.

• With knowledge of over 200 different NDIS providers in the Geelong area, The SC can prepare referrals to your chosen providers.

• SC continues an ongoing working relationship with the participant and their chosen providers. The SC can hold care team meetings to assist the participant and their providers to stay on track with providing relevant and effective services to their goals.

• The SC regularly reviews services with the participant and assists in making changes when necessary or when requested by the participant when exercising their choice and control.

• SC keeps all the participant’s documentation and relevant information obtained during the plan duration in a secure filing system i.e. service agreements, reports, support letters, etc.
