Regular communication
Geelong Support Coordination knows that each client values being up to date and given answers to their questions. This has formed a big part of GSC operations and what level of service is provided.
Services and tasks requiring regular communication can include the following:
SC reviews each individual NDIS plan and explains what a NDIS plan is to the participant and what has been allocated to them and what that can look like in terms of what services they can get and what limitations have been set by the NDIA.
With the participant’s goals in mind, the SC explores potential services with the participant and addresses each budget in the participant’s NDIS plan (More details about budget tracking and the answers this can provide can be found on the next page).
After the first 8 weeks of the plan the SC writes a “8-week progress report” and submits it to the NDIA to give an update about how the initial implementation stage is going and whether there needs to be some tweaks made to the plan. If the plan is inadequate to meet the participants goals, then this is also a good time when the SC can assist the participant in submitting a “reviewable decision” to the NDIA to request an early plan review in an attempt to adjust their plan funding and gain a more reasonable and necessary NDIS plan.
Risk assessments
The SC Identifies risks: Mental health (inc. Self-harm/suicide), Drug & alcohol, Aggression, Forensic (involved in justice system), Physical health, financial (i.e. gambling issues, managed by state trustees), Housing, Environmental (i.e. dogs at property, housemates) and can develop a safety plan with the participant.
