Geelong Support Coordination service agreement
GSC provides each participant with a Service Agreement which states all the terms and conditions for a honest and transparent service. This agreement must be completed and signed by both parties before further service begins.

Code of Conduct
GSC operates with the NDIS code of conduct which promotes safe and ethical service delivery and requires workers and providers delivering NDIS supports to:
act with respect for individual rights to freedom of expression, self-determination, and decision-making in accordance with relevant laws and conventions
respect the privacy of people with disability
provide supports and services in a safe and competent manner with care and skill
act with integrity, honesty, and transparency
promptly take steps to raise and act on concerns about matters that might have an impact on the quality and safety of supports provided to people with disability
take all reasonable steps to prevent and respond to all forms of violence, exploitation, neglect, and abuse
take all reasonable steps to prevent sexual misconduct.

NDIS Worker Screening Check
All GSC employees undergo all necessary checks and trainings including the NDIS worker screening check and ‘Quality, Safety and You’ - NDIS Worker Orientation Module.
Obtain consent to share information:
GSC does not share your private information without your consent. The SC can provide you with relevant consent forms required for the job, such as:
The NDIS consent to share information form which gives permission to your chosen providers to talk to the NDIA regarding private details about the participant such as the details on your NDIS plan.
The GSC consent to share information with other providers form (this is our own document which states which providers and main stream services the participant gives consent for us to talk to and share information about their relevant needs, ex. If the Support Coordinator needs to share details of your NDIS plan with other providers, GP, DFFS, justice system, Community case workers, etc).
